What is PRIME SAN?
The PRIME Support and Alignment Network is part of the landmark federal, Transforming Clinical Practice Initiative, within the Centers for Medicare and Medicaid Services' Innovation Center. This initiative was designed to provide clinicians free technical assistance to build the comprehensive quality improvement strategies needed to survive and thrive in a value-based reimbursement environment.
Over the next three years, through enrollment in one of twenty-nine Practice Transformation Networks, clinicians will have access to the extensive nationwide, collaborative CMS Quality network, which includes the thirty-nine Practice Transformation and Support and Alignment Networks of the Transforming Clinical Practice Initiative.
The American Board of Family Medicine and the American Academy of Family Physicians are teamed up in the PRIME Support and Alignment Network, offering the following benefits to help support and align family physicians' professional activities with their practice improvement efforts:
Free abfm PRIME Registry Dashboard Subscription
Free PRIME Registry subscription for up to 3 years to the first 6,000 ABFM board-certified family physicians who sign up and are enrolled in a Practice Transformation Network. The PRIME Registry is open to all primary care providers (including NPs and PAs) and includes a robust set of clinical quality measures calculated at the clinician level, practice level, and down to the individual patient level. View your measures in your secure, web browser-based PRIME Registry dashboard. Clinicians may also authorize PRIME to automatically report their clinical quality measures to federal initiatives such as PQRS, TCPI, MIPS, and APMs to come.
ABFM Family Medicine Certification-Performance Improvement activity credit is available upon attestation for enrolling and engaging with your Practice Transformation Network team. It is awarded for each year you are enrolled and engaged with your Practice Transformation Network and requires annual attestation to receive credit.
The attestation includes a short explanation and answer format about your PTN participation. Upon completion of the two-part attestation, you will receive Performance Improvement activity credit (formerly called “MC-FP Part IV credit”) towards your Family Medicine Certification requirements.
To apply for your credit, log in to your Physician Portfolio, find Spotlight Programs on the bottom left of the page, and select the PTN Participation tab to begin your attestation. We have created a step-by-step instructional tool (PDF) to assist your attestation process as well.
*Note that Residents are able to receive 20 Family Medicine Certification points for completion of the attestation. As part of their requirements this attestation will not fulfill the “minimum of one (1) Performance Improvement Activity with patient population.” The 20 points will, however, help Residents reach their minimum 50 points requirement.

Sign up to join Family Physicians with shared interests in the Transforming Clinical Practice Initiative Member Interest Group. There, AAFP members connect for peer support and learning, to encourage Practice Transformation Network engagement, and for in-person collaboration at the Family Medicine Experience (FMX) conference, the AAFP’s largest annual meeting of Family Physicians.
Continuing Medical Education series for credit
Three Transforming Clinical Practice Initiative related sessions presented live at FMX, the annual Family Medicine conference. Offering Family Physicians CME and collaborative learning sessions to support the transition to value-based reimbursement.
*Note: Your practice is not eligible to enroll in a PTN if you currently participate in any of the following: Medicare Shared Savings Program (MSSP), Pioneer Accountable Care Organizations (ACOs) including the Next Generation ACO model, the CMS Comprehensive Primary Care Initiative (CPCI), or the CMS Multi-Payer Advanced Primary Care Practice (MAPCP) ACOs. You may, however, join a PTN if you are part of a Medicaid or private payer ACO program.
